OUR VISION
FOUR LEVELS OF CARE
ROUTINE HOME CARE
Care is provided in a home setting, this could be your own house or a facility. Our caring staff will visit you, and Galaxy Hospice will provide all medical equipment, supplies, and medications related to the primary diagnosis.
CONTINUOUS CARE
When a medical crisis develops which cannot be managed by the family, and symptoms cannot be controlled with scheduled intermittent visits, Galaxy Hospice will coordinate and manage continuous home care until patient’s condition stabilizes. Care is available for as much as 24 hours a day as necessary to maintain patient at home.
GENERAL INPATIENT CARE
When care is needed for the treatment of acute medical and/or psychosocial symptoms related to the terminal illness, inpatient care on a short-term basis is available at a contracted skilled nursing facility or an acute care hospital
RESPITE CARE
If needed, Galaxy Hospice can provide a brief inpatient stay to allow a caregiver or family member a “respite” or break.
